Rhinoplasty operations are always done at the operating room, in a clinic or a hospital. Normally, patients will remain in the clinic overnight after the operation. Next day, the patient is discharged. Rhinoplasty operations can be done under local anesthesia with sedation, or, more frequently, with general anesthesia, depending on the complexity of the operation and the preferences of your surgeon. If done with local anesthesia and sedation, the patient feels relaxed and the nose and surrounding areas will not sense any pain; with general anesthesia the patient is asleep during the operation. Rhinoplasty usually lasts between 1 and 2 hours, although in complex cases it can take longer. During surgery, the skin is separated from the nose, composed of bone and cartilage, and sculptured in the desired way. Finally the skin is redistributed over the new nose. Many surgeons operate from inside the nose, making little incisions inside the nostrils; others prefer open Rhinoplasty , specially for the more complex cases, in which a little incision in the columella is made. After surgery, a plaster, which will help maintain the shape of the nose, is placed. Nose plugs in both nostrils will also be placed to avoid bleeding and stabilize the nasal septum.
BACK TO NORMALITY
After surgery, and especially after the first 24 hours, you can feel your face swollen, nose discomfort and headaches, which will cease with medication. You must lay down with your head up for the first day (except for going to the toilet). You will notice that the swollenness and hematomas around the eyes will reach a maximum after 2 or 3 days. Cold compresses will decrease the swollenness and make you feel better. Most of the hematomas and the swollenness will disappear in 2 weeks (a light swollenness, possibly only perceptible for your surgeon, could last a few months). A little nose bleeding during the first days is normal. You should not blow your nose strongly during the first week, more or less, until the tissues cicatrize. If you have nose plugs, these will be removed in 1 or 2 days, after which you will feel much better. After a week, maximum two, the plaster will be removed, and the stiches, if you have any. Most of the patients that undergo a rhinoplasty feel well after 2 days, being able to return to work or to school in a week or less. Anyhow, you must wait a few weeks in order to do some activities. Even though your maxillofacial surgeon will instruct you, here are some recommendations: avoid extenuating activities (footing, swimming, pushups…) during 2 or 3 weeks, protect your nose from bangs and from the sun for 8 weeks, be careful washing your face and applying makeup. You can use your contact lenses as soon as you want. If you use glasses you most take some precautions. Your maxillofacial surgeon will be seeing you during the following weeks of the operation for supervision. For any doubts you may have or any discomfort between visits, do not hesitate to call him.

Anyhow, there are always possibilities of complications like infection, nose bleeding or reactions to the anesthesia. Risks can be minimized following carefully your surgeons instructions, both before and after the operation. Occasionally, after surgery small red dots can appear in your skin, they are broken blood vessels; generally they are very small, but they are permanent. There are no problems related with the scars, as incisions are made inside the nose, even in the open rhinoplasty the scar is imperceptible. Approximately, in 1 out of 10 cases it is necessary a surgical revision to correct minor defects. These cases are not predictable and occur even to the patients of the most experienced surgeons.
RESULTS
During the first days after surgery, when the face is swollen and with hematomas it is easy to forget that our appearance is going to improve and a lot of patients can fell a little depressed; it is a normal feeling. Day after day, appearance improves and the patient starts to feel happier. In 1 or 2 weeks, you will not appear to have undergone surgery. The Improvement is progressive and gradual, even though a minimal swollenness can remain, especially in the tip, for a few months. Definitive result are obtained one year after the operation approximately.
SUMMARY

After the intervention, which usually lasts about one hour, a nasal plug is applied, which is usually removed the next day, except in the case of a septoplasty, in which case it will be removed after 2 or 3 days. A plaster will also be placed over the nose and must be kept on for a week. Postoperative edema and the presence of ecchymosis (hematoma) the first week are expected. Most of the patients can go back to normal life after 8-10 days. Judgment over definitive results should await several months, even a year. This is the time needed for the tissues to stabilize and for the scars to complete the process of maturation. The intention is that the “new” nose does not stand out, and fully integrates in the general context of the face producing a nice and harmonic conjunct. In other words, it does not appear to have been intervened. On some occasions, due to important initial anatomic alterations, or to disorders in the tissue healing, due to postoperative traumatisms, etc.., perfect result is not obtained (asymmetries, irregularities of the nasal surroundings ,etc…) a small surgical operation may be necessary, in which case it will be carried out after six months.
Blepharoplasty, the surgery of the eyelids, is a procedure to extract fat and the excess of skin and muscle of the lower and upper eyelids. Generally patients are 35 years or older, but in some cases of patients with genetic tendency to have bags, patients are younger. Before deciding to undergo blepharoplasty think about what you want to achieve and discuss it with your maxillofacial surgeon. Some medical conditions can increase the risks of undergoing a blepharoplasty, like a thyroid problem, dry eye, high blood pressure, heart problems and diabetes. Glaucoma and fallen retina are also a risk, in determined cases, an eye examination may be required previous to the blepharoplasty.
THE SURGERY
Good communication between you and your maxillofacial surgeon is essential. In the first visit the eyesight and tear production will be evaluated together with your general state of health. You must inform your surgeon of any medication you may be taking, of smoking habits and about any known allergies. You must also inform if you wear glasses or contact lenses and hand in, if possible, your last eye examination. The possibility of operating the four eyelids, upper and inferior ones, and if it is necessary to associate this operation with another procedure will be discussed. The techniques and the anesthesia used will be explained to you, where the operation will take place, the risks and the costs of the intervention. Aesthetic Blepharoplasty is not covered by the Social Welfare. Do not hesitate to ask your surgeon about any doubts you may have, especially those related to your expectations and results. Your maxillofacial surgeon will provide you with specific instructions on how to prepare for surgery, including rules for eating and drinking, tobacco, medicines and vitamins, and washing your face. It is important to stop smoking at least one or two weeks before surgery. A careful compliance will ensure surgery to be carried out under the best conditions. Make sure, that some relative or friend can take you home when you are discharged, and, if necessary, can help you for a few days. Blepharoplasty usually lasts between 1 and 3 hours, or more if done along with other interventions. When the four eyelids are going to be operated, the superior ones are usually intervened first. Most of the times the incisions are made in the natural lines of the superior eyelids and just under the eyelashes in the inferior ones; in some cases, they can extend towards the crow´s feet. Through these incisions the skin is separated from the fat and the muscle under it, extirpating the excess of fat and, on occasions, the excess of skin and muscle. The incisions are closed with very thin suture. In other cases, in young patients with only an excess of fat, a transconjunctival blepharoplasty of the inferior eyelids can be done, the incision is made in the inside of the inferior eyelash, leaving no visible scar.
BACK TO NORMALITY
After surgery, pomade will be applied to the eyes to lubricate them, and, in some cases, a soft bandage. You may notice discomfort over the eyelids, which will cease with medication prescribed by your surgeon (contact your surgeon if you suffer persistent or strong pain). You must maintain your head up for the first days after the operation. Cold compresses will decrease the swollenness and the hematomas. You will be instructed on how to wash your eyes and you will be indicated if you have to use ophthalmic drops to maintain your eyes hydrated. During the first weeks you may notice excessive tears, sensibility to light and temporal changes in visual acuity, like blurry or double sight. During the first weeks your plastic surgeon will follow closely your evolution. Stiches will be removed between 2 and 7 days after surgery. Edema and hematomas will decrease gradually until completely disappearing, and you will start looking and feeling much better. Most of the patients that undergo a blepharoplasty start reading and watching television in 2 or 3 days. However, he will not be able to put in his contact lenses, if he uses them, until 2 weeks after, and might feel uncomfortable for a while. You usually return to work a week or ten days after, by then, you will be able to put on make up to cover the hematomas. It can be necessary to wear sunglasses for a few weeks and apply solar filter on the eyelids. You must rest for 3 to 5 days and avoid strong activities for 3 weeks.

However, there are always possibilities of complications like infection or reactions to the anesthesia. Risks can be minimized following carefully your surgeons instructions, both before and after the operation. Minor complications that can appear after a blepharoplasty are blurry or double vision for days, temporal edema of the eyelids and slight asymmetries in healing. After surgery, there could be difficulty in completely closing the eyelids during sleep; only in rare cases this complication could be permanent. Another rare complication is ectropion, which may require surgical correction.
RESULTS
Scars may have a pink appearance during the first months. They will gradually disappear until turning into an almost invisible faint line. The results of a blepharoplasty are a younger and more awake appearance, which will remain for years and in a lot of cases, the results are permanent.
Esthetic Blepharoplasty is the operation to correct the signs in the eyelids which manifest facial ageing: drooping eyelids and “bags” in the inferior ones and in the inner portion of the superior ones. The intervention can be practiced as a separate procedure or as part of another operation, in general a facial lifting. Depending on this, the intervention will be practiced under local or general anesthesia. The operation is initiated with incisions in the skin of the eyelids in order to liberate and extirpate the excess of skin and orbicular muscle. Next, the correct amount of fat is extirpated to eliminate the palpebral bags. Finally, the skin is sutured. After the intervention, which lasts approximately one and a half hours, an ophthalmologic pomade is applied and some bandage with ophthalmologic drip very cold in order to reduce the discomfort and inflammation. However, inflammation and hematomas in the eyelids during the first weeks are normal. Most of the patients can go back to their normal lives in 10-12 days. At the beginning the scars may be visible, like fine red lines situated in the fold of the superior eyelid and under the eyelashes in the inferior eyelid, extending a few millimeters towards the crow`s feet. Sometimes, after a blepharoplasty, eyesight might suffer for 2-3 weeks. Conjunctivitis and subconjunctival hematoma can also appear and will quickly disappear with the proper treatment. During the first days it can happen that during sleep the eyelids will not close completely, this is normal, and when the orbicular muscle is recuperated( eventually helped by the right exercises) it will be corrected. The result of the operation is favorable and long lasting and, in general, the bags in the eyelids do not reappear.
Otoplasty is surgery that can replace prominent ears, or protruding ears to a more normal position, closer to the head, or to reduce the size of the ears. Generally, it is surgery performed on children from 4 to 14 years. Ears are fully developed by the age of 4, therefore, the sooner surgery is practiced, the less time the child will suffer ridicule at school.
Otoplasty can also be performed on adults, without major risks.
If you are considering undergoing otoplasty, you will obtain basic information about the surgery, when it is indicated, how it is done and what results can be expected. Besides prominent or protruding ears, there are other problems that can also be solved with surgery. These include cryptopia and constricted ear. Earlobes can also be surgically improved. WHO IS THE IDEAL CANDIDATE TO UNDERGO AN OTOPLASTY ?
Most surgeons recommend parents to observe the child´s behavior regarding his prominent ears; You must not insist on surgery until it is the child`s desire. Those children who are unhappy with their ears and are willing to be operated, are more cooperative during the intervention and are happier with the results.
Many patients, children or adults, can be insecure about the results of the otoplasty. Keep in mind that the objective is to obtain an improvement not absolute perfection. Don´t expect an exact symmetry, it might even look unnatural. If you discuss with your maxillofacial surgeon, the procedure, your expectations and options, results will be very satisfactory.
On your first visit the plastic surgeon will make an evaluation of the problem and will recommend the more effective technique in your case. He will also provide you with instructions on how to prepare for surgery. Esthetic otoplasty is not covered by the social welfare, except in those cases in which there is an important psychological trauma or the patient suffers a severe deformity.
SURGERY
Otoplasty is carried out in an operating room inside a clinic or a hospital. Depending on the anesthesia the procedure se realize en regimen ambulante or the patient will remain in the hospital overnight. If the child is small the surgeon will recommend you to have the operation under general anesthetia . For older children and adults, local anesthesia, with or without sedation, will be used. Therefore, the patient will be awake during surgery, but relaxed.
Otoplasy usually lasts between two or three hours although in complex cases it can last longer.
The technique used will depend on the specific problem. Generally a little incision is made on the back of the ear to expose the auricular cartilage. Afterwards the cartilage is sculptured and folded backwards; internal stitches can be used to maintain the new form.
On occasions, it may be necessary to extirpate part of the cartilage in order to obtain a more natural ear. Finally a piece of skin from behind the ear will be extirpated. The resulting scar will be hidden behind the ear. Most of the times, even though only one ear might look abnormal, both will be intervened to achieve a better symmetry.
BACK TO NORMALITY
Most of the patients, both children and adults, feel fine a few hours after surgery, although it may be advisable to stay in the clinic overnight until the effects of the general anesthetic have disappeared. A bandage is placed around the head of the patient just after the operation. The ears can hurt a little the first few days, but will be easily alleviated with medication. In a few days the bandage will be exchanged for a lighter one similar to a hair band. It is important to follow the instructions of your maxillofacial surgeon regarding the use of the bandage, especially at night. During the first month any activity which would involve folding the ears should be avoided. Most of the adults go back to work five days after surgery; children go back to school after seven days approximately once they are careful with physical activities.

When otoplasy is performed by a qualified maxillofacial surgeon complications are infrequent and of little importance. However, like in any other intervention, there are surgical risks. A small percentage of patients can develop an hematoma that may dissolve spontaneously or may need drainage. Occasionally, an infection of the cartilage may occur, which will increase the healing tissue of the ear; its treatment consists on the administration of anti-biotics. It is a surgical intervention to repair the wrinkled skin of the face and neck caused by age, poor diet or hereditary tendencies. It involves removing excess fat, tensing the main muscles and refilling the skin of the face and neck.
DESCRIPTION
Wrinkles in the skin appear as the years go by especially in the area surrounding the mouth and nose, crow´s feet, between the eyebrows, etc. Deposits of fat appear around the neck, the skin around the chin sags (double chin). Other factors that can contribute to problems with the skin in younger people are poor diet, tobacco, obesity and other hereditary factors. Lifting helps improve visible damage to the skin and muscles, recuperating a younger appearance. Lifting can also be done together with a rhinoplasty or bleforaplasty. Lifting can be done with two types of anesthesia; local anesthesia where the patient is sedated and doesn´t feel pain or general anesthesia where the patient is asleep during the procedure.
The plastic surgeon makes some incisions along the hairline in the temples, behind the ear lobes and the lower area of the scalp. After he extracts the fat tissue, tenses the skin, and finally, stitches the incisions.
INDICATIONS
Normally it is performed to reduce the signs of ageing (wrinkles), which upset the patient. This can even create health problems. Most of the patients are happy with the results. Sometimes the surgeon has to place small drainage tube under the skin from behind the ear to drain a small quantity of blood that can accumulate in this area. Some compressive bandages are placed around the head, to avoid inflammation and bruises (hematomas). Normally the patient doesn´t feel very uncomfortable after the operation. In any case medication can be prescribed for the pain. The patient may notice numbness; this is normal and it disappears after a few weeks. The patient´s head must be kept elevated with at least two cushions (minimum angle 30º) to avoid inflammation. The drainage tube (if necessary) will be removed one or two days after the intervention. Bandages will be removed after one to five days. The face may appear pale, bruised or swollen but will recover its normal appearance in four to six weeks. Most of the stitches will be removed in five days, although some of the stitches located at the edge of the scalp are left for a few days more because they take longer to heal.
RISKS
For 10 to 15 days the patient will suffer inflammation, tightness, numbness, hematomas, etc.. Most of the scars are hidden under the hairline and disappear after months. Sunbaths should be avoided.

Bichat ball, also known as the Buccal fat bag, is a fat pad located inside the cheek under the bone. It occupies a well defined space in this area where we do not find any important facial structures. It is a well defined anatomical structure and can be extracted easily.
THE OPERATION
The intervention can be performed under local anesthesia with sedation or general anesthesia. When performed alone hospitalization is not necessary.
A small incision is made inside the mouth next to the molars. The ball is extracted through this incision, stitching afterwards the incision area.
RECOVERY
Inflammation will decrease to normal levels after a week, and the edema(swelling) and cheek inflammation will decrease progressively. Any possible complications will disappear around one month after surgery.

PLANIFICATION OF SURGERY
It is important that your expectations are well understood. We will help you to determine if the treatment and objectives you propose are possible. It may be of help if you provide pictures of people with similar features to the ones you would like to have. You must be sure that you have understood the details of the proposed surgery, including the cost and the healing and post operative process that awaits you. If surgery includes incisions in the mouth, it is important to inform if you smoke or about any dental problems you may suffer. In such case, you will be informed about the risks and treatment required previous to the operation. Make sure to arrange for somebody to drive you to and from surgery and to stay with you for at least the first night after the intervention. You must also follow our indications about eating and drinking before and after surgery, and also about any medication you may be taking, including aspirins or other similar drugs. WHERE DOES SURGERY TAKE PLACE
These kinds of interventions take place at the hospital, generally on an outpatient basis. We will inform you if you need to stay in the hospital overnight. ANESTHESIA

Cheek surgery usually lasts between 30 and 45 minutes. When performed combined with other cosmetic procedures, like facial lifting, frontal lifting or eyelid surgery, the implants can be inserted through incisions made for these operations. FACIAL IMPLANT SURGERY RECOVERY
You will be informed about recovery procedures. There will be limitations on the diet and normal activity will be restricted. However, these instructions vary, especially when another procedure is combined with the implants. Your capacity to move your mouth and lips can be affected temporarily. The stitches used to close the incisions will either be removed after 10 days or will dissolve progressively.
RECOVERY AFTER SURGERY
Edema (swelling) is sometimes considerable immediately after surgery, and generally reaches a maximum degree 24 or 48 hours after surgery. Most of the inflammation will disappear in a few days, but a slight edema (swelling) may persist, not being possible to appreciate the resulting facial contour, until a few months after. During the healing process, your diet and activities will be restricted. Your capacity to smile, speak or move your lips will be limited for a few days or weeks after surgery. We will inform you of the dental and oral hygienic measures to apply during recovery.
BACK TO NORMALITY
Remember that any facial surgery can make you feel better and improve your appearance in a short amount of time. However, it may not be recommendable to participate in certain activities for some weeks, especially in those involving the treated areas.
YOUR NEW APPEARANCE
It is possible that you are not able to appreciate your final appearance until weeks or months after the operation. This is because the body needs a time to eliminate completely the inflammation. Be patient when evaluating your definitive appearance. You will be surprised to see that most people do not notice the new implants, only an improvement in your appearance.
